STEP #1. |
As soon as
you have finished the required number of
hours of firefighter training, fill out the
upper section of the form.
It is available to download from this
website OR through the mail by calling
1-800-308-5336 or 405-424-1452 |

STEP #2. |
After you
have signed and dated the form in the upper
right, have your Fire Chief sign the form,
below the line in Part A of Section 2. |

STEP #3. |
 Make a copy
for yourself and one for the department's
files.

Your
application will be processed, training
verified, and COFT will certify it. Then, it
will be mailed back to you at the address
